Job Description Full-Time Nurse Practitioner (NP) – Des Moines, IA – Integrated Kidney Care We are looking for a Full-Time Nurse Practitioner (NP) to join our Integrated Kidney Care team in Des Moines, IA. This position offers a self-managed schedule with no nights, weekends, or on-call responsibilities. About the Role As an Integrated Kidney Care Nurse Practitioner (NP), you will support complex patients and help them navigate the healthcare system. You’ll deliver holistic, integrated care focused on improving the quality of life for kidney patients, while also handling primary care responsibilities to address their overall healthcare needs. This role allows you to work autonomously while collaborating with a multidisciplinary healthcare team to coordinate quality, cost-effective care. Position Details Primary Care Responsibilities: - Perform routine health maintenance exams, wellness checks, and preventive care screenings. - Manage chronic conditions such as hypertension, diabetes, hyperlipidemia, CHF, asthma, and COPD within the context of kidney care. - Educate patients on lifestyle modifications, preventive care strategies, and chronic disease management. - Prescribe and monitor medications, adjusting treatment plans as necessary. - Collaborate with healthcare providers and manage referrals to specialists. - Ensure timely follow-up for preventive care and chronic disease management, including vaccinations, screenings, and routine labs. - Complete comprehensive health assessments. - Focus on reducing hospitalizations and improving health outcomes for complex patients. Schedule: - Monday through Friday with flexibility to accommodate patient and partner availability. - Full-time, self-managed schedule. - No nights, weekends, or on-call. Compensation & Benefits - Competitive salary - Comprehensive benefits package - CEU/CME reimbursement - Paid license renewals - Additional benefits available Requirements - Passion for delivering care to patients with complex, chronic illnesses. - Minimum of two (2) years of experience as a Nurse Practitioner. - Current unrestricted NP license to practice in the state. - Current DEA license (or eligible to obtain within 90 days of hire). - Current CPR or BLS certification. - Intermediate computer skills (MS Word, Excel, Outlook). - Home office setup with high-speed internet and reliable transportation. Preferred Qualifications - Experience with value-based care models, including Medicare Advantage risk adjustment and health risk assessments. - 5 years of experience as a Family Nurse Practitioner, Primary Care NP, or in Internal Medicine, Cardiology, or Nephrology. - Spanish-speaking preferred in some markets. - Experience managing chronic diseases such as CHF, diabetes, hypertension, and others common in primary care. - Experience managing population health with a focus on reducing hospitalizations and improving chronic disease outcomes.
