We value our employees and believe they are the core of our organization.Purpose:Responsible for supervising assigned care staff, clinical activities and works to ensure care provided in the home is coordinated and managed appropriately according to state and federal guidelines for home health care.Responsibilities:Assesses and supervises the delivery of home care services for an assigned group of patients receiving home care.Performs supervisory visits of nursing services delivered by licensed nurses and home health aides to patients living at home.Reports changes in the patient's medical or mental condition to the physician and the Clinical Manager.Reviews patient's clinical diagnosis, medications, procedures and care needs.Reviews care documentation for accurate and timely completion.Assists care team with establishing immediate and long-term therapeutic goals for patients receiving care.Provides assessment and oversight of individual care plans for assigned group of patients; (i.e. private duty and Non-skilled patients).Assists the Clinical Manager with duties related to supervision of staff, care coordination and oversight of care.Facilitates resolution for patient and caregiver matters that arise.Provides skilled nursing services by making home visits for patients at home and providing direct nursing care.Provides daily oversight of staffing and manages issues that arise.Observes staff perform skills for clinical care procedures to ensure competence and/or confidence when providing care to patients.Ensures care staff receiving the training needed for performing care services.Ensures compliance with policies and procedures.Implements quality improvement activities for responding to patient satisfaction, compliance, completed care documentation, revenue cycle, incident reporting, and complaints.Completes care documentation quality audits as assigned.Supervises the day-to-day operations for the private duty/nonskilled services.Ensures compliance with all requirements and reporting expectations.Facilitates onboarding, orientation, training, and continuing education for supporting the learning of all care staff.Performs in-home supervisory visits with field staff for compliance and performance.Maintains an open line of communication with all levels of staff.Serves in the on-call rotation as assigned.Performs other duties as assigned.Qualifications:Current Iowa registered nurse license in good standing requiredThree years clinical experience working as a registered nurse in a healthcare setting required; prior home health experience is preferredAt least one year health care supervisory and/or management experience preferredBSN degree preferredPossess excellent critical thinking and decision-making skills as well as strong nursing assessment skills.Computer proficiency with working knowledge of Microsoft Office, including Word, Excel, and Outlook.Time management skills with the ability to effectively plan, organize and prioritize work.Ability to communicate effectively, both verbally and in writing.Current Healthcare Provider CPR certification required.Must have valid drivers license, auto liability insurance and reliable transportation if driving is required.Successful completion of pre-employment physical, drug screen, 2-step TB test and background check.Working Conditions:Physical requirements: Medium work exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.Physical activities: Climbing, balancing, stooping, kneeling, crouching, crawling, reaching, standing, walking, pushing, pulling, lifting, talking, fingering (picking, pinching, typing, etc.), hearing, grasping (applying pressure with fingers and palm), repetitive motions, feeling (perceiving size, shape, texture, temperature by touching), visual acuity (field of vision for analyzing data, viewing computer, using measurement devices, operating machines, determining accuracy or neatness, and/or making general observations)Environment: Extreme temperatures, Hazards (chemicals, infectious disease), atmospheric conditions (fumes, odors, dusts, mists, gases)Time: Ability to be flexible with a varying schedule and programmatic requirementsTravel: Required between locationsThese characteristics are representative of those an employee encounters while performing the essential functions of this job.
